Onsite Biometric Screenings

Each year, you must complete a biometric screening and online health risk assessment to be eligible for the Medical Plan Wellness Premiums for the following plan year. See Medical Pre-Tax Premiums Per Pay Period on the Medical Plans page for more information.

Complete an onsite biometric screening:

When: 9 a.m. – 1 p.m.
Where: 6465 Mullins Station Road, Memphis & Shelby County Code Enforcement Building

You don’t need an appointment. Screenings are available on a first come, first served basis for up to 75 employees.

After you complete your screening, you must submit the screening form to Cigna — onsite staff will not submit this for you. View instructions for submitting the form by mail, fax, or electronically on the upper right side of the screening form.
